Key findings and diagnosis – A large, isodense to arterial structures ovoid lesion is present at the suprasellar cistern on the left extending into the sella. There is abnormal increased density to the CSF, layered blood in the occipital horns and prepontine and interpeduncular cisterns, and mild obstructive hydrocephalus. The appearance is concerning for a sentinel bleed from a large right ICA aneurysm
